NEW YORK: Jefferson County establishments pay fines for violations in July

The state Department of Health reported the following violations and fines for July:
■ Grand Buffet, LeRay, paid $800 in fines, including a $600 fine because a tobacco smoke odor was detected in the air and ashes were found on the floor of the employee restroom, which was a repeat violation. The establishment also paid two $100 fines because cooked chicken was cooled at an inadequate temperature and there were inadequate hand-washing facilities in the dish-washing area.
■ Original Italian Pizza, Watertown, paid $450 in fines, including a $250 fine because the facility was conducting business without a valid permit to operate during time of inspection. Two $100 fines were paid because pizza topped with various potentially hazardous food products was stored at an improper temperature, and there were no adequate facilities for holding pizzas topped with potentially hazardous food products.
■ Carvel Ice Cream, LeRay, paid $600 in fines, including a $300 fine because the facility was in operation without a valid permit to operate, a repeat violation from 2010. The establishment also paid $200 because large cans were severely dented and therefore not protected from potential spillage or contamination. Two $50 fines were paid because the hand-washing sink in the employee restroom was inaccessible for hand washing and cold water was not available in the hand-wash sink in the back food preparation area.
■ Robert’s Family Pizzeria, Watertown, paid a $100 fine because the facility was operating without a valid permit and a complete application for a permit had not been received by the department.
■ Subway, Public Square, paid $800 in fines, including a $500 fine because the facility was being operated without a valid permit to operate, a repeat violation from 2010. The establishment also paid a $30 fine because at the time of inspection the $75 annual environmental health service fee had not been submitted to the Watertown District Office. That was also a repeat violation from 2010.
■ Teriyaki Experience, Towne Center plaza, paid $350 in fines, including three $100 fines because at the time of inspection, the facility was operating without a valid permit to operate; the annual Environmental health services fee had not been received by the department and a food service worker did not wash his or her hands after cleaning tables and floors in the dining area. The establishment also paid a $50 fine because ventilation hoods and devices were not free of grease and dust.
■ Sandy Acres Community, Adams, paid a $250 fine because it was being operated as a mobile home park without a valid permit to operate.
■ Nu-Bren-Way, Wilna, paid a $300 fine and a $50 surcharge because on July 6 a pack of Marlboro cigarettes was sold to a 17-year-old.
■ Ontario Place Hotel, Sackets Harbor, paid $300 in fines, including a $200 fine because the facility was operating without a valid permit to operate, a repeat violation from 2010. The establishment also paid a $100 fine because at the time of inspection the required environmental service fee of $100 had not been paid to the department.
■ Showtime Skating, Watertown, paid a $100 fine because the facility was operating without a valid permit to operate at the time of inspection and a complete application for a permit had not been received by the department.
